چکیده

Abstract Background and aim Standard single antiplatelet regimen (SAPT) is currently recommended before invasive risk stratification in non-ST elevation acute coronary syndromes (NSTE-ACS). However, there are subsets whom dual therapy (DAPT) may be more protective according to the anatomy revascularization strategy. We aimed identify pre-procedural predictors of artery complexity that helpful selecting an individualized regimen. Methodology Retrospective single-center study including patients with NSTE-ACS performing angiography between January 2020 July 2021. Clinical variables classical scores (TIMI, GRACE HEART) were captured SYNTAX 1 score was calculated. Patients divided into low (0–22), moderate (23–32) high (≥33) score. Multivariate logistic regression analysis performed determine anatomical (defined as significant left main disease [stenosis ≥50%] or ≥33). Results A total 448 included (mean age 67±13 years; 74% males; 85% NSTE-myocardial infarction). Overall, 350 (78%), 63 (14%) 35 (8%) had a low, high-SYNTAX score, respectively. Thirty-one (7%) disease. At multivariate (adjusted for age, diabetes, renal function, TIMI scores), HEART (hazard ratio 2.3, 95% confidence interval 1.5–3.3, p<0.001) predicted complexity. When ROC curve analysis, discriminative ability identifying those complex (area under 0.79, CI 0.72–0.86, (Figure 1) – >7 showed sensitivity 76% specificity 75% such patients. Conclusion In this cohort patients, independent anatomies. These results suggest (>7) merit from SAPT opposed DAPT, given probability lesions amenable CABG. Serum neutrophil gelatinase-associated lipocalin levels are correlated with the complexity and the severity of atherosclerosis in acute coronary syndrome

OBJECTIVE Neutrophil gelatinase-associated lipocalin (NGAL) is a novel inflammatory marker that is released from neutrophils. In this study, we evaluated the correlation between serum NGAL level and clinical and angiographic risk scores in patients diagnosed with non-ST elevation acute coronary syndrome (NSTE-ACS). METHODS Forty-seven random NSTE-ACS patients and 45 patients with normal coron...

TIMI, PURSUIT, and GRACE risk scores: sustained prognostic value and interaction with revascularization in NSTE-ACS.

AIMS Regarding prognosis, patients with a non-ST elevation acute coronary syndrome (ACS) are a very heterogeneous population, with varying risks of early and long-term adverse events. Early risk stratification at admission seems to be essential for a tailored therapeutic strategy. We sought to compare the prognostic value of three ACS risk scores (RSs) and their ability to predict benefit from ...

Coronary lesion complexity assessed by SYNTAX score in 256-slice dual-source MDCT angiography.

PURPOSE The SYNTAX Score (SS) has an important role in grading the complexity of coronary artery disease (CAD) in patients undergoing revascularization. Noninvasive determination of SS prior to invasive coronary angiography (ICA) might optimize patient management. We aimed to evaluate the agreement between ICA and multidetector computed tomography (MDCT) while testing the diagnostic effectivene...

Management of Patients With NSTE-ACS

Ma Non–ST-segment elevation acute coronary syndromes (NSTE-ACS) are the leading cause of morbidity and mortality from cardiovascular disease worldwide. The American Heart Association/American College of Cardiology and the European Society of Cardiology periodically release practice guidelines to guide clinicians in the management of NSTE-ACS, most recently in in 2014 and 2015, respectively. The...

Accuracy of the TIMI and GRACE scores in predicting coronary disease in patients with non-ST-elevation acute coronary syndrome.

INTRODUCTION The GRACE and TIMI scores have been well validated for assessment of prognosis in non-ST-elevation acute coronary syndrome (NSTE-ACS). However, their value in predicting coronary artery disease (CAD) has been little studied. We aimed to assess the relationship between these scores and the extent of coronary disease. METHODS We analyzed 238 consecutive patients admitted for NSTE-A...
